Free HRT in Ireland: What It Covers
Since 1 June 2025, hormone replacement therapy (HRT) has been available free of charge in Ireland for many women. The scheme covers the cost of the medicine itself. It does not cover the consultation where HRT is discussed and prescribed.
Who can get free HRT?
You can get HRT free of charge at a participating pharmacy if you live in Ireland and you have either:
- a medical card, or
- a Drugs Payment Scheme (DPS) card.
The scheme applies to women experiencing perimenopause or menopause symptoms. There is no age limit, so it also includes early, premature and medically induced menopause.
If you don't have a medical card, it is worth registering for the Drugs Payment Scheme. Registration is free, and you need to be registered before your HRT can be dispensed free of charge.
What does the scheme cover?
The scheme covers the HRT medicine itself when it has been prescribed for you. This includes the main types of HRT, such as:
- tablets
- skin patches
- gels and sprays
- vaginal creams and pessaries
Not every product is on the list. If you have been prescribed a specific product, your pharmacist can tell you whether it is covered.
The medicine must be collected from a pharmacy that takes part in the scheme. The HSE publishes a list of participating pharmacies you can search.
What isn't covered?
The scheme pays for the medicine, not for medical advice. It does not cover:
- private consultations with a GP or menopause doctor
- follow-up or review appointments
- private blood tests or other investigations
Medical card holders can see their own GP free of charge. Other patients pay their usual consultation fee, whether they see their own GP or a private menopause clinic.
Why a consultation still matters
Free HRT removes the cost of the medicine, but it doesn't change the most important question: which treatment, if any, is right for you.
HRT comes in different types, doses and routes. The right choice depends on your symptoms, your medical and family history, whether you still have a womb, and your own priorities. Some women need their treatment adjusted over the first few months; others have symptoms that are better explained by something other than menopause.
A consultation gives time to go through this properly, discuss the benefits and risks for you, and agree a plan with regular reviews.
The medicine may be free. Getting the right medicine, at the right dose, is what the consultation is for.
How to get free HRT
- Make sure you have a medical card or are registered for the Drugs Payment Scheme.
- Discuss your symptoms with a doctor, who can assess whether HRT is suitable and write a prescription.
- Bring your prescription to a pharmacy that takes part in the scheme.
If you are unsure whether you qualify, your pharmacist can check for you.
